  Having an issue with the contact form. When I click on the tab it is completely blank.

  Go to Settings > Theme Settings to set up your contact form, under the “Contact Form” heading.

  Note that in your contact form content that you included in your post you have added two contact forms. One uses the default fields, the other uses custom fields. Use one or the other, not both.
